Where an accident or incident resulting in injury has occurred, a work place is required under the act to undertake actions to reduce the risk of a similar accident or incident occurring.
In the event of an accident, injury, incident or near miss it is usually the Site Controllers (Manager or Supervisor) responsibility to conduct an “Accident or Incident Investigation” as soon as possible after the event.
Further training may be required for personnel, this can be undertaken by issuing a safety alert in relation to the incident, carrying out a toolbox talk, and training in Safe Operating Procedures.
